We have a full-time vacancy for a Leaving Care Case Worker which is a post that is fundamental to delivering our Core Offer to Care Leavers and ensuring the best outcomes for our young people leaving care.  You will manage a caseload of young people, to act as a point of contact, to provide essential advice, advocacy, and support and to make a lasting impact.

 

Based in Scarborough, you will ensure the assessment and review of the needs of young people in, and who have left public care, as well as planning and providing support to meet their assessed needs. As a Leaving Care Case Worker, you will be a confident communicator and act as a role model to the young people you represent through the advice and support you provide. This may include assisting care leavers with adjusting to the next stage of their life such as moving into accommodation, help with budgeting and household management, as well as assistance with education, training, and employment.

 

You will be committed to providing positive outcomes for care leavers by completing essential documents to map out their future journey such as a Pathway Plan. You will then become their key worker once the young person has left care.
