This job can be based at any of our offices across the county. The role offers the flexibility of hybrid working from an agreed office base.

 

As part of our commitment to reducing the number of people who experience a long stay in hospital, we are recruiting a Social Care Coordinator to work within in the Craven locality area. If you are an experienced worker looking for a new challenge, we hope you will want to join us.

 

Reporting to North Yorkshire Council, Health and Adult Services, Intermediate Care Operations Managers and Advanced Practitioners . You will work closely with our social workers, social care coordinators, therapists, nurses and healthcare support workers towards preventing admission to hospital and facilitating early discharge to a range of care settings. 

 

You will be required to work at pace, supporting short-term interventions with people, their families and carers. This may involve undertaking a range of assessments and developing personalised care and support plans that enable people who are unwell or in a health crisis to remain in their own homes.

 

You will determine service eligibility for adults with care and support needs against the national standards and ensure that patients and carers are safe from harm. Where harm or abuse has been identified, you will support the team to progress Safeguarding work.

 

This is a wonderful opportunity to be part of local service developments and improvements designed to deliver integrated care for people. You will work closely with our health partners with opportunities to work in hospital and the wider community.  You will work closely with our community social care teams to ensure that people receive appropriate support in the longer term.
