Would you like to play a key role in improving the health, wellbeing and independence of adults across North Yorkshire? An exciting opportunity has arisen for a team manager to join our Targeted Prevention Service on a full-time basis.
Our Living Well service forms part of our wider Targeted Prevention service, as a service we help people to access and connect with their local community, and support them to find their own solutions to their health and wellbeing goals, helping to reduce loneliness and isolation. Our living well coordinators work with individuals and their carers who are isolated, vulnerable, bereaved, lacking confidence, or perhaps on the borderline of needing health and social care services.
As a living well manager, you will be responsible for providing day-to-day line management to a team of Living Well Coordinators. An ambassador for NYCC, you will lead, develop, coach and mentor your team to ensure our services are high quality and meet our vision and ambitions. You will be fundamental in shaping the management and direction of our service as you instil a culture of continuous improvement across the service, actively working with the staff team to deliver excellence.
This role is critical to the successful delivery of the prevention agenda and increasing people’s independence, implementing the elements of the Care Act on prevention and reducing the impact on health and social care services. In delivering this agenda, you will work with a range of partner agencies across the county including, public health services, stronger communities within the council in addition to the NHS, District and Borough Councils, the community, voluntary and independent sector and user-led organisations.
We ask that you have a Level 4 / NVQ 4 or equivalent / QCF qualification in a relevant area and an enhanced DBS clearance is required for this post. You will manage the Living Well Coordinators within the geographical area of Hambleton and Richmondshire and you will provide additional manager support for the Harrogate Craven team 7 hrs. per week.
