Are you an experienced Social Worker ready to take the next step? Join us as an Advanced Practitioner and help shape the future of adult social care across the community in our specialist Mental Health Team.

 

 

In this pivotal role, you will:

 

  • Lead on quality assurance, safeguarding coordination, and complex casework
  • Support managers to embed strength-based, relationship-focused practice
  • Mentor and coach colleagues, driving best practice and performance
  • Manage a small team of Social Care Coordinators and Wellbeing Officers

 

 

We are looking for someone who brings:

 

Expertise in safeguarding, risk assessment, Mental Capacity Act (MCA) and Best Interest decision‑making

You will provide confident, evidence‑based professional judgement to support safe and person‑centred practice.

 

A passion for improving outcomes for adults and carers

You are driven by making a real difference, promoting independence, dignity, wellbeing, and positive experiences for the people we support.

 

Strong leadership and practice development skills

You will champion high standards, mentor colleagues, and help shape a culture of learning, reflection, and continuous improvement.
